Job summary
This is an excellent opportunity for highly motivated and enthusiastic
Band 5 Community Staff nurses or those wanting to gain invaluable experience of
community nursing to join the Community Integrated teams in Sunderland working
flexibly over a seven day period to cover 08:00 to 18:00 Hours We are seeking to recruit full and
part-time posts. Applicants are invited for those due to qualify in sept 2024.
These
posts are ideal for somebody who would like the opportunity to experience
community work or develop their interests in the community setting further. The
role incorporates the day to day management of patients within the community
under the supervision of senior nurses, involving a variety of skills including
wound care, continence management, palliative care, health promotion,
venepuncture and more. Training will be available and candidates will be
supported with an induction and a personal development plan. Full support will
be given to those without community experience to gain the necessary skills
required.
Main duties of the job
Community
Nurses are required to provide a range of skilled, holistic nursing care to
patients in their own homes, and within wound care clinics in various
localities across Sunderland. You will work within a Community Integrated Team
consisting of District Nursing Staff, Community Practitioners, Specialist
Nurses for Older People, Social Services and third sector organisations. You will deliver high quality nursing care,
advice, support and monitoring to patients within the community, participating
in local innovations in practice and care delivery and be expected to identify
and assess the core needs of patients and carers. You will work collaboratively
to deliver identified packages of care for patients with complex and non-complex
needs with the aim of promoting independence and self-management where possible
and deliver palliative and end of life care to those who choose to die at home.
Successful
applicants will work under the direction and supervision and be line managed and
accountable to a senior nurse in
the Integrated Team.
You
must also be able to meet the travel requirements of the post and be a car
owner and driver.
