Job summary
This is an exciting
opportunity for Band 5 Registered Nurses to work as part of the NEW Community
Ready Unit at South Tyneside District Hospital. This is a 14 bedded unit
designed to support patients who are experiencing a delay in their discharge
home from hospital, for example awaiting commencement of a care package.
As
a nurse led unit the registered nurses will be responsible for the planning,
assessment and evaluation of any of the patients nursing care needs such as
wound management, catheter care etc. liaising with the responsible Doctor where
there are medication needs or episodes of minor illness.
The
units philosophy builds on the 'Home First ' principles as these patients are
ready for home and no longer in need of acute hospital care. The team will be
supporting patients to increase their independence, prompting the patients with
self -administration of medication, supporting mobilisation, and independence
with dressing and other activities of daily living whilst they wait to go home.
Main duties of the job
To be
responsible and accountable for the delivery of safe, effective and
compassionate nursing care for patients designated to their care.
To work
effectively within a multi-disciplinary team ensuring patients receive
appropriate interventions to be able to improve their health and maximise their
level of independence.
To always
work to the NMC Code
To
supervise non-registered staff as stipulated by the NMC under the requirements
of delegation
Provide
effective leadership and assist with the management and organisation of the
unit/department in the absence of the Ward manager or deputy
To
actively support safe and timely discharge for patients to their own home /
permanent residence
To comply
with the trusts vision, values and behavioural compact
