This is an exciting opportunity for a Learning Disabilities and
Autism Acute Liaison Practitioner to join the team at South Tyneside and
Sunderland Foundation Trust working with the Community and Acute Learning
Disabilities and Autism teams. You will be part of a team that works together to change lives for
the better, developing specialist skills and making a real difference for some
of the most vulnerable people in society.

The post holder will work Monday to Friday and work between South
Tyneside Hospital and Sunderland Royal sites and be expected to meet the travel
requirements for the post across the Trust sites and off-site meetings as required.
Main duties of the job
As a Learning
Disabilities and Autism Acute Liaison Practitioner , you will ensure people who
have a learning disability and if they also have autism to receive high-quality
care, while also supporting your colleagues and promoting positive teamwork.
As
part of a team you will:
- Work
in collaboration with community learning disabilities teams, care providers,
families and carers to enhance patient safety, providing support and advice
meeting the patient's needs whilst in hospital.
- Provide
specialist clinical care and advice to patients with a learning disability and
autism or both, and to the professionals who support them offering advice and
guidance on Health Passports and Reasonable Adjustments required to gain best
outcomes for the patient.
- Provide
information around the process of mental capacity and best interest decision
making.
- Support
a safe discharge.
- Devise and deliver training to acute
colleagues regarding supporting adults with learning disabilities and autism
and the role of reasonable adjustments and the liaison nurse.
