are looking for highly skilled, enthusiastic, dynamic self-motivated physiotherapist
to join our Community Falls Team based at Washington Galleries.
Would you enjoy the challenge of adopting a
MDT approach to providing assessment and interventions to reduce the risk of
falls in the delivery of high-quality care for the patients in the community
setting.
If

You will be responsible to provide care to people
within the clinic setting, undertake comprehensive holistic assessment of patients,
be able to prioritise clinical need, initiating investigations, determine a
differential diagnosis and implement a plan of care reflecting evidence-based
guidance in conjunction with NICE guidance.
Main duties of the job
Community Falls Team working hours
08.30-4.30 Monday - Friday.
Trust adheres to flexible working

You will adopt an MDT approach and undertake
triage within the service determining clinical priority of patients and assess,
design, and deliver specialised interventions, exercise and education
programmes for patients referred to the Falls service within the clinic setting.
