Job summary

An exciting opportunity has arisen for an enthusiastic, well-motivated and flexible Junior Nurse Practitioner to join our Urgent Community Response Service, Recovery at Home in South Tyneside, based in Clarendon in Hebburn

The team consist of an extensive MDT which includes nurse practitioners, nurses, occupational therapists, physiotherapists, health care assistants, therapy assistant/exercise practitioners and administrative staff. The team also works closely alongside partner agencies including those from the local and voluntary sector.

This role would involve assessing people in their own homes who are either acutely unwell or have recently been discharged from hospital and require enhanced intervention and short term support for their physical and functional health needs to remain at home.

The service is fast paced and you would be expected to see a wide range of patients who present with a variety of conditions.

This is a unique and rewarding role which will allow you to work with patients within their usual place of residence to enable them to remain there safely alongside you developing your own clinical skills.

Main duties of the job

The service is committed to providing a sound environment for education and training to support the successful candidate within this role.

The post holder will need to be proactive with their own learning and work to identified timescales

You will possess the following

1st level Nurse with significant post registration experience

Degree or can demonstrate on a degree pathway

Accredited Clinical Skills qualification

Portfolio of evidence of competence and continuous professional development.

Leadership qualification/training

  • To provide continuing responsibility in the absence of the Senior Nurse Practitioner and act in their absence
