Job summary
Hospital at Home are looking for a staff nurse who is interested in the post for a the OPS hospital at home service. We are looking for a qualified nurse who is motivated and enthusiastic to join our developing service and multi-disciplinary team, with a passion for acute community care.
Hospital at Home is a service alongside virtual wards specialities within Older Persons Services. Our aim is to provide excellent person-centred holistic care to our service users and their families.
The role will include assessments and implementation of plans of care to patients whom conditions and treatment can be met within their own home or within the community settings safety to prevent them from remaining within the acute hospital settings when deemed appropriate.
Use of a car for transportation is essential.
