Patient Safety Investigation and Liaison Lead

Job summary

We are looking for a part-time patient safety investigation and liaison lead to support learning from patient safety events and compassionate engagement.

These roles are instrumental in University Hospitals Dorset's (UHD) implementation of the Patient Safety Incident Response Framework (PSIRF). The role of investigator is to be an expert in Patient Safety Incident Investigations (PSII), recognising the complexities of healthcare and enabling teams and organisations to learn from when care hasn't happened as expected. PSIIs are in-depth investigations requiring both excellent investigative skills as well as excellent communication skills with staff at all levels within the organisation and with those affected by incidents.

Base Location: Royal Bournemouth and Poole Hospital. Cross site working will be required.

Main duties of the job

The post holder will investigate patient safety incidents under the PSIRF, acting as a role model and expert advisor for staff involved in the reviews and ensuring that all reviews are carried out to the highest possible standards.

The post holder will undertake Board level PSIIs using SEIPS methodology in alignment with UHDs PSIRF policy and Plan. This will involve reviewing highly complex clinical information, and drafting a chronology of events. The investigator will need to interview staff at all levels of the organisation and undertake site observations. They will be expected to engage with patients and their carers/families which can involve managing potentially highly emotive conversations dependent on the incident being investigated. A key part of the framework is ensuring that feedback is given to patients and their carers/families, as well as sharing the findings to the staff involved and the Trust Board.

The investigator will provide education in learning responses and investigation and the principles of Duty of Candour and patient involvement and engagement, ensuring understanding and following of best practice in relation to PSIRF and the principles of Duty of Candour.

The post holder will be expected to be a credible member of the leadership team within Quality and Safety, which means deputising for the Head of Patient Safety as required.
