Job summary

Ward Two is currently looking to recruit a proactive and patient centred Discharge Coordinator. As a ward we prides ourselves on our effective team work. As a discharge Coordinator on Ward Two, you will be required to interact closely with all members of the Multi-Disciplinary Team and other external health and social care services in order to facilitate safe and effective discharges for our respiratory patients. Base Location: Bournemouth Interview Date: TBC

  • NVQ level 3: health / social care management or equivalent

Experience

  • Experience of working in a hospital setting
  • Understanding of complex discharge planning in care of the elderly, stroke, long term conditions and vulnerable adults
  • Experience of supporting service users specialised care needs to enable informed decision making

Technical Skills Competencies

  • Use of IT including Windows, Word, Excel, Powerpoint,
  • Assessment, planning and organisational skills

Knowledge

  • Knowledge complex discharge planning process.
  • Understanding of NSF's, CHC Framework, Delayed Discharged Act and Reimbursement.
