Job summary

Hospital at Home are looking for a motivated and enthusiastic Health Care Support Worker to join our newly developing service and friendly multi-disciplinary team.

Hospital at Home is a service alongside virtual wards specialities within Older Persons Services. Our aim is to provide excellent person-centred holistic care to our service users and their families.

Use of a car for transportation is essential. The role involves lone working at times.

This post is being advertised as a Band 3, however applicants that do not meet the required care certificate competencies will initially be employed as a Band 2. Applicants who hold a care certificate accreditation will start in post at Band 3.

Main duties of the job

The role will include implementation of care and on-going assessments of patients whom conditions and treatment can be met within their own home or within the community settings safety to prevent them from remaining within the acute settings when deemed appropriate.
