Job summary

An exciting opportunity has arisen to join an existing UHD multidisciplinary team, working on the Poole and Bournemouth sites. The aim of the service is to care for patients accessing UHD services who are experiencing homelessness or at risk of homelessness. The role will also involve the care of patients undergoing treatment for Chronic Hepatitis C across Dorset as part of the Dorset Hepatitis C Elimination Programme.

Our patient group often have multiple complex care needs and face health inequalities and issues with accessing care due to drug and alcohol addiction issues, homelessness and mental health problems.

We are looking to appoint an individual with a background in care administration and coordination, with a desire to be more patient facing.

The ideal candidate will also have knowledge and experience related to enabling patients to access local authority housing, including emergency/temporary housing.

Base Location: Bournemouth

Main duties of the job

The post holder will work with the Homeless Care team of nurses to advocate for patients, coordinate care and effectively engage patients with complex needs undergoing care at UHD who are experiencing homelessness or at risk of homelessness and also undergoing treatment for Chronic Hepatitis C.

  • NVQ level 3: health / social care management or equivalent
  • 5 GCSE (to include English & Maths)/or relevant experience

  • 1st Level Degree, or working towards
