Job summary

Community Heart Failure Nurse

Dorset Community Heart Failure Service is seeking to recruit a motivated and compassionate Heart Failure Nurse. You will work as part of a well-established, experienced, and dynamic heart failure nursing team covering Mid and North Dorset.

You will provide high-quality, patient-centred care to individuals living with heart failure in community settings, supporting early intervention, education, and long-term condition management.

This is a part time role of 30 hours per week. The post holder will need to be flexible to meet the needs of the service.

Main duties of the job

The post holder will work autonomously as an advanced practitioner and have continuous responsibility for proactively managing a caseload of patients with heart failure in the Community.

Deliver expert, evidence-based nursing care to patients with heart failure across community settings.

Undertake comprehensive holistic assessments, including clinical examination, interpretation of investigations, and risk stratification.

Optimise heart failure therapy in line with NICE and local guidelines, including independent prescribing and medication titration.

Manage complex and unstable patients, making autonomous clinical decisions within scope of practice
