Come and join the Community Support Workers Team in the North. We are looking for Band 3 Support Workers to support us develop and adapt to best meet the care needs of our local population. We are committed to improving career development and training opportunities.
You will be providing an essential service to our local population to prevent avoidable hospital admissions, enable earlier hospital discharges and care for patients whose preferred place to die is their home.
Professional support and access to the Trust's clinical supervision, in-service training and other developmental opportunities are just some of the benefits of working as part of this team. The other benefits involve working in a beautiful area of Dorset as part of a large friendly team, who are co-located with numerous other community teams including the District Nursing team, Hospital at home team, Community Rehab team, Frailty team, Community Mental Health Team and Social Services team.
A caring attitude, access to a car and driving licence, an excellent communicator and enjoy working as part of a team as well as competent with lone working is essential for the role.

Main duties of the job
The post will involve treatment of patients in their own homes. Our Community Support Workers work with guidance and support from our qualified Therapy and Nursing staff.
This post would suit a motivated individual who enjoys a busy working day, with a varied caseload.
ICRT service covers 08:00 - 18:00, seven days a week. This post is for someone who is interested in full time or part time hours with a set rota shift pattern. We are committed to enabling a good work-life balance so some flexibility and requests may be considered providing the needs of the service can be met.
As a community post access to a car and drivers licence is essential.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
