Job summary

We are currently seeking a dynamic, enthusiastic, Community Staff Nurse to come and join our vibrant and innovative North Bournemouth Frailty Team (NBAT) to support the health of our community in Bournemouth North PCN.

This is a part-time permanent post of 30 hours per week. Working week Monday to Friday (To include Tuesday and Friday). The post holder may be required to work flexibly to meet the needs of the service.

As a community staff nurse, you will be providing nursing care to patient in their own home. We provide a wide and varied level of care to patient in their home. This means we have highly skilled workforce, successful candidates will be supported with upskilling and training.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

Main duties of the job

As a community staff nurse, you will provide a high-quality person-centred approach to care delivery which always considers people's safety, privacy, and dignity.

As a member of the multi-disciplinary team, you will be participating in yearly reviews for housebound patients with long-term chronic conditions QOF.

Undertake assessment of patients and service users care needs and develop programmes of care as part of the wider multi-disciplinary care team and signposting to another services and voluntary sector as appropriate.

Utilise validated assessment tools such as, Frailty Index, 6CIT (Six-Item Cognitive Impairment Test), Pressure Ulcer Risk Assessments, Other Dorset Healthcare University NHS Foundation Trust (DHUFT) tools as required.

Perform clinical procedures within scope of practice, including, Venepuncture (obtaining bloods), Electrocardiograms (ECGs). Other relevant clinical interventions as appropriate to role and training.

Provide evidence-based advice to patients, carers, and families regarding health conditions and care plans. To plan, implement and review health improvement programmes in a range of settings.

Facilitate sensitive conversations around future care needs, including, advanced care planning, end-of-life discussions. Preferences for treatment and care settings.

Ensure timely and accurate documentation of clinical visits and interventions using System One.

Provide supervision, mentorship, and clinical support to students and junior staff in the team.
