We're looking for a motivated and enthusiastic Band 6 Physiotherapist to join our dynamic Weymouth and Portland Integrated Community Rehabilitation Team --part of the Integrated Neighbourhood Team (INT)--delivering care where it matters most: in the heart of the community
Your Role:
Deliver high-quality Physiotherapy assessments and treatments to patients with acute illness.Prevent unnecessary hospital admissions and support complex discharges.Help patients achieve long-term goals--primarily in the community, with occasional work on the ward at the local Community Hospital.

Requirements:
HCPC registered Physiotherapist.Valid UK Driving Licence and access to a car is essential.Passion for community-based care and rehabilitation.

Main duties of the job
To provide Physiotherapy assessment and treatment to people in their own homes, care homes or on Community Hospital wards, dependant on Locality.
To provide a high-quality person-centred approach to care delivery which always considers people's safety, privacy and dignity.To provide specialist care and interventions, assessments, treatments and managing patients on a caseload, as part of the wider multi-disciplinary care team.
To provide specialist advice to others regarding the management and care of patients / service users.
To devise specialist programmes of care for other professionals to deliver and ensure they are delivered appropriately.Act as a role model and promote the Trust Behaviours of being proactive, positive, respectful, supportive, reliable and trustworthy.
