Senior Intermediate Care Physiotherapist

Job summary

We are looking for an enthusiastic Band 6 Physiotherapist to join our multidisciplinary Intermediate Care Team based within the Poole Locality in Dorset.

The role is to provide Physiotherapy assessments and treatment to patients with acute illness to prevent unnecessary admission to hospital and facilitate complex discharges from hospitals, working within the community with patients in their own homes as part of a hospital at home service.

The successful candidate will need to hold a valid UK Driving License and have access to a car.

Although this is a contract for 30 hours, there may be additional hours that can be offered on a temporary basis up to full time.

As a Band 6 Physiotherapist you will have the opportunity to:

Develop your discipline specific skills.

Develop skills in holistic assessing.

Develop your supervisory, leadership and service development skills.

There will be opportunity to support junior, assistant staff and students to help with their development

Main duties of the job

To provide Physiotherapy assessment and treatment to people in their own homes.

To provide a high-quality person-centred approach to care delivery which always considers people's safety, privacy and dignity.To provide specialist care and interventions, assessments, treatments and managing patients on a caseload, as part of the wider multi-disciplinary care team.

To provide specialist advice to others regarding the management and care of patients / service users.

To devise specialist programmes of care for other professionals to deliver and ensure they are delivered appropriately.Act as a role model and promote the Trust Behaviours of being proactive, positive, respectful, supportive, reliable and trustworthy.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
