Job summary

The Christchurch Older Persons Community Mental Health Team are looking for a experienced Community Mental Health Nurse to join this team.

As a Mental Health Nurse, you will provide a comprehensive accessible and responsive mental health service to adults and their families within a defined locality. This is primarily a care-coordination role, where you will be responsible for supporting patients on their journey of recovery. You will be a committed, dynamic, motivated professional with excellent communication, organisational skills and the ability to prioritise your work. You should have relevant and recent experience of working in a mental health environment and be able to apply the philosophies of engagement and recovery in your work with patients, their carers and their family.

You will work as part of a well-established supportive team made up of a Psychiatrist, Nurses, Psychologists, a Social Worker, Administration and a Support Time Recovery Worker. We offer peer support led by psychology, and regular management and clinical supervision to enable you to feel safe in your practice whilst further developing your skills.

This role is for 28hrs 15mins and you will need to have a valid driving licence and use of a car, as you will be required to assess and monitor patients in their own homes and within other community settings.

Main duties of the job

To provide a comprehensive accessible and responsive mental health service to Older adults and their families within a defined locality.

To support the team leader and the locality service manager in implementation of the Dorset Healthcare Mental Health Strategy.

To continually develop clinical, team and service evaluation and delivery of service.

To contribute to effective multi-disciplinary working within the community mental health service and to build up effective liaison with community, primary, and other secondary and tertiary services/agencies within the locality to provide holistic care based on the individual's need and service development.
