Access Wellbeing-Single Point of Access Co-ordinator

Job summary

The new single point of access for Access Wellbeing Weymouth is looking for a Coordinator role.

This is an exciting and new developing role. It is a pivotal role to bring services together and provide overall oversight and administrative functions across the whole of Access Wellbeing Weymouth.

It is an exciting opportunity to be at the forefront of transformation developments in Weymouth.

Working closely with our on site team, including colleagues in Primary Care, Secondary Care and Universal Services you will work to help people access the right support at the right time for their mental health and wellbeing.

This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.

Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position.

Main duties of the job

This role is a critical and evolving post to support Access Wellbeing.

To implement and run a daily single point of access (SPOA) meeting in the locality to co-ordinate and action all requests for specialist and focussed community mental health support and intervention in the locality.

To oversee and track patients' requests - collating all relevant information to support the SPOA panel to reach an appropriate decision for patients and then tracking progress through services.

Monitor the performance and effectiveness of the SPOA.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
