Community Diabetes Specialist Nurse

Job summary

We are seeking an enthusiastic registered nurse with some experience in diabetes to join our dynamic team. The role involves clinical management of people with diabetes in a variety of community settings, including community clinics, care homes, and home settings. The community diabetes specialist nurse team has a strong commitment to delivering excellent diabetes care and knowledge.

This role is essential in providing expert care, education, and support to patients with diabetes across the community setting.

You will work autonomously and collaboratively to assess, plan, implement, and evaluate person centred care. The post holder will play a critical role in empowering individuals to manage their condition effectively, improving outcomes and quality of life.

Main duties of the job

  • Deliver advanced nursing care for patients with diabetes in the community.
  • Conduct holistic assessments and create individualised care plans.
  • Educate patients, families, carers on diabetes self-management and prevention.
  • Provide expert guidance to primary care teams and other community professionals.
  • Support continuity of care through seamless coordination between services.
  • Educate and upskill healthcare professionals on diabetes management and care.
