Job summary
Within the Christchurch & Southbourne Community Mental Health Team, 1 position has arisen for a Band 6 Community Mental Health Practitioner working on a part-time basis.
The successful candidate will work as part of an integrated team made up of Psychiatrists, Nurses, Psychologists, Employment Specialists, Physical Health Support Workers, Social Workers, Administration and Support Time Recovery Workers. This is primarily a care-coordination role, where you will be responsible for supporting patients on their journey of recovery.
As a key member of the team, you will find the experience challenging, but rewarding and varied within the well-established and supportive team, where we have strong values about working in a patient centred manner, where their recovery and wellbeing is at the heart of everything we do. You can expect close working peer support and regular supervision to enable you to feel safe in your practice whilst further developing your skills.

Main duties of the job
You will be a committed, dynamic, motivated professional with excellent communication, organisational skills and the ability to prioritise your work. You should have recent experience of working in a mental health environment and be able to apply the philosophies of engagement and recovery in your work with service users.

You will contribute to effective multi-disciplinary working within the community mental health service and to build up effective liaison with community, primary, and other secondary and tertiary services/agencies within the locality to provide holistic care based on the individual's need and service development.
The team is based close to Christchurch Hospital and BCP Council Locality Office in the BH23 region; the post holder will need to have the ability to travel by car, with business insurance, as they will be required to assess and monitor patients in their own homes and within other community or inpatient settings.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
