Primary Care Pharmacist - Fixed Term (for maternity cover)

Job summary

The Senior Pharmacist will undertake clinical medication reviews and patient facing clinics, working in partnership with the patient's GP practice, nurses, care homes, community pharmacists, secondary care and the local authority.

Utilising their prescribing qualification, the role will support the medication needs of the patients as well as educate other healthcare professionals on prescribing and the effective use of medicines.

The successful applicant will need to be self-motivated and able to work on their own, whilst also integrating into a well-established team. You will have excellent organisational, interpersonal and communication skills together with a good working knowledge of primary care and the challenges facing medicines management in the new era.

This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application.

Main duties of the job

Main duties include:

  • Medicines optimisation working closely with practice management to deliver key prescribing objectives in structured medication review
  • Care home clinical services
  • Antimicrobial stewardship
  • Personalised pharmaceutical care, transfer of care
  • Integrated care with community services
  • Prescribing safety and governance.
  • Audits e.g. High risk drug monitoring

Knowledge, skills and training

Essential

  • Vocational masters degree in Pharmacy (4 years or equivalent) plus one year pre-registration training
  • Registered with the General Pharmaceutical Council
  • In depth therapeutic and clinical knowledge and understanding of the principles of evidence based healthcare

Desirable

  • Postgraduate clinical pharmacy diploma or equivalent experience.
  • A member of or working towards Faculty membership of the Royal Pharmaceutical Society

Job specific experience

Essential

  • An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing
  • Evidence of involvement in the development of programmes of care, protocols and audit
  • Experience of working in a multidisciplinary team

Desirable

  • Have an understanding and working knowledge of the FP10 data management system (ePACT).
  • Experience of co-ordinating complex activities / programmes of care with other professionals and agencies.
