Job summary
Dorset HealthCare University NHS Foundation Trust are seeking to recruit a Band 6 Community Mental Health Practitioner to work in Weymouth for the Community Mental Health Team for Older People and join our well established team. If you are a RMN or OT with experience working with people suffering severe mental illness whether this be inpatient or community based we would love to hear from you.
The Service is a specialist team providing assessment, treatment and support for individuals suffering from organic and functional mental health illness. Experience of working with older adults with organic / functional mental illness in an inpatient or community setting would be an advantage. The ideal candidate will be enthusiastic, self-motivated and hard-working, with a positive approach to delivering high quality care to the older adult and be willing to work collaboratively with carers and other professionals. You will carry a defined caseload and manage this with the support of the team leader and the widerteam.
There will be opportunities for you to develop your own clinical assessment skills, whilst risk assessment and management will remain at the forefront of everything you do. Being a learning organisation, we are happy to support you in advancing your own skill set using additional learning opportunities such and NMP, ACP, and psychological therapies etc depending upon organisational and service demand.
Main duties of the job
We work with a multitude of services across the Weymouth and Portland area to ensure the mental health needs of the older adult population is met. We work a duty rota within the team, this is set over a 6-month period, so each week the duty persons day changes.
This job is challenging, being a very busy CMHT so if you're ready for this then we are looking for someone who is reliable, committed and motivated with excellent communication and organisational skills and the ability to prioritise their caseload balancing service demands. In return we are ready to offer you comprehensive and structured management supervision, management you can approach any time and peer support which is second to none.
The trust has a robust Crisis and Home Treatment Team and Intermediate Care Service for Dementia (ICSD), both based in Rural West Dorset. Additional support is provided by the local mental health charity with peer run drop in centres and education courses. We work closely with the local IAPT services.
The post holders will work as part of an established integrated multi-disciplinary team. This team is made up of a Psychiatrists, Psychologists, Nursing staff and Support Time Recovery Workers.
The primary role responsibility is to act as dedicated Care Coordinator for a caseload of patients with whom you would provide person centred, holistic care to assist in aiding their wellbeing and recovery.
