Job summary
Are you an experienced clinical leader with strong leadership skills? The Dorset ICS are seeking a driven Cluster Lead to support with progressing our vision for Home First delivery. This post, employed by Dorset Healthcare on behalf of the ICS, will be required to work across the system to achieve the strategic ambitions of Home First, and Discharge to Assess.
You will be part of a team providing a leadership role to locality clusters across Dorset. You will have a focus on East Place area, enabling and facilitating timely discharge across pathways, providing an interface with community teams, and sharing information regarding previous community involvement with acute partners, with a requirement to provide an element of in reach support to our acute partners University Hospitals Dorset and Dorset County Hospital, while working in partnership with Local Authority colleagues.
You will need to utilise clinical experience of out of hospital care with leadership skills to maximise the opportunity for admission avoidance, flow, assessment following discharge, and successful completion of patient journeys through discharge pathways using a strength-based approach. This is a great opportunity for learning and development working alongside an experienced team.
The Cluster Lead post is a full-time, fixed term / secondment position. We welcome part time applications with a minimum of 30 hours. We can be flexible with how these hours are met which can be discussed at interview.
Main duties of the job
This role is currently fixed term until 31st March 2025 with the intention that this period is used to evaluate and inform the transition to and adoption of a new Transfer of Care Hub model. Those with current NHS service will be offered on a secondment basis only. Please obtain permission to be released on a secondment before applying.
To provide clear visible leadership within the service area. To act at all times as an advocate for service users, carers and relatives.
To maintain competency and ensure own compliance with mandatory training and revalidation requirements. Identify own personal development and educational needs to work at an advanced level ensuring appropriate action is taken to maintain and further develop such skills.
Act as a role model demonstrating effective leadership on a daily basis and promote the Trust Behaviours of being proactive, positive, respectful, supportive, reliable and trustworthy.
Take the lead in ensuring that home first pathways are delivered and escalating where increase in referrals or any potential to breach pathways may occur.
To ensure that all adverse incidents, complaints and PALS enquiries received are investigated in line with Trust policy; to respond to complaints appropriately and within agreed timescales, action plans formulated and actioned, feedback given to staff and ensuring outcomes are reported to the Locality Director or Locality Manager as appropriate.
