Job summary

Are you an empathetic, forward-thinking nurse who can adapt to changing priorities and deliver high quality nursing care?The Community Learning Disability in Ferndown and Purbeck are looking for you to join!

You will be able to demonstrate their commitment to work proactively and in partnership with people with learning disabilities, their families and carers and support them to live successfully in the community. This role requires you to use your nursing knowledge and experience to support individuals within the community andto maintain effective links with mainstream services, working in partnership with colleagues and supporting equal access to healthcare. You will be committed to improving outcomes for our service users and have excellent interpersonal and communication skills, in supporting the health needs and health promotion to support and empower people with a Learning disability.

The Community Learning Disability Team (Ferndown and Purbeck) is staffed by a passionate and dedicated team. This is an excellent opportunity to practice within the challenging yet rewarding field of community learning disability nursing whilst working alongside a multi-disciplinary team with our existing nurses, trainee nurse associate as well as other members across our area.

The Community Learning Disabilities Nurse post is a full-time Monday to Friday role. As this is a community post being a driver is essential for this role. For any queries, please contact sean.farrow@nhs.net

Main duties of the job

To provide support and work directly with people with learning disabilities in their home, including working with their families and Care & Support providers.

To ensure all practice is based upon the principles of Valuing People; rights, independence, choice and inclusion. To work with the individual, their carers and members of the multi professional team in the formulation, implementation and review of a person-centred care and support plan. This will include risk assessments and management plans that will outline the support the individual requires to continue to live successfully within the community.

To support a wide range of staff who work in social services and the independent sector providing clinical expertise and advice in aspects of behaviour and learning disability raising their awareness to ensure they meet the specific needs of the service user.

To promote the health needs of the individual and provide health facilitation support to them; this will also include supporting them in the development of their health action plan.
