Job summary

  • To provide medical cover to Swanage Community Hospital Inpatients and Purbeck Hub
  • Ward patients fit into 3 categories: step-down patients from local acute hospitals, requiring rehabilitation; step-up patients from the community, requiring investigation and management of medical problems; End of life care.
  • Rehabilitation is focused on maximising individual potential, with the aim of helping patients return to their own home.
  • Swanage Hospital also provides expert End of Life care for local patients unable to be cared for at home in their final days.
  • The work of the doctor is supported Advanced Clinical Practitioners, both on the wards and in the community.
  • Working within a MDT (ACPs, ICRT, DN, and Social Services) to provide clinical support patients, to maintain them in their own place of residence where possible.
  • To work with and support the Development of the Purbeck Virtual ward, attend virtual care meetings, providing medical advice as required.
  • To work closely with the ACP team lead in the future development of the team in regard to quality care and governance.
  • To work flexible across the Hub and ward as required and if appropriate support the step up of community patients to CoHo wards.
  • To develop a working relationship with the Primary Care team in Purbeck.

Main duties of the job

  • The work of the doctor is supported by an additional doctor who covers 2 days per week at the site in addition to Advanced Nurse Practitioners, both on the wards and in the community.
  • The intention is that the post holder will maintain good contact with local GP practices, including undertaking domiciliary visits for patients referred for hospital.
  • Medical input/advice for inpatients and the Community Rehabilitation Team.
  • Domiciliary review of patients as part of integrated community working and post-discharge follow-up.
  • Support to the multidisciplinary team, including nurses and allied health professionals, in developing extended
  • Weekly ward round by consultant geriatrician and phone support from same consultant/consultant on call
