The Weymouth & Portland Community Learning Disability Team (CTLD) have an employment opportunity! Are you are an enthusiastic, forward thinking nurse who can adapt to changing priorities and deliver high quality nursing care then we are looking for a like-minded nurse to join us.

This is an excellent opportunity to practice within the challenging yet rewarding field of community learning disability nursing. This is a full-time post, but we would be prepared to agree a reduced number of hours for the successful candidate (30 hours or more).

We are part of an integrated team based with our local authority colleagues. The team takes referrals for adults with learning disabilities across the Weymouth & Portland area. We work with people to promote a healthy lifestyle and reduce health inequalities. We aim to ensure that people enjoy the highest possible quality of life.

Our team comprises of nurses, physiotherapists, occupational therapists and nurse apprentices and we have close links to psychiatrists, speech and language therapists and clinical psychologists, both in LD services and more broadly. We work closely with social workers employed by Dorset Council, working together to provide the best outcomes.

We also work closely with local care providers, supporting them to monitor and enhance their delivery of care including bespoke training and supervision. We are frequently involved in complex transition cases where young people are moving from children's to adult services.

Main duties of the job

The team use their specialist clinical skills and experience to assess, plan care, offer treatment and review, working closely alongside colleagues from other disciplines, external care providers as well as the individual and their families.

The team uses a combination of virtual and in-person contacts with professionals and service users alike.

Clinical documentation is in electronic form and training is given to all new staff.

Working hours are Monday - Friday 9am to 5pm, with some flexibility to meet individual need.
