Job summary

Are you a Band 6 Occupational Therapist who has an interest in working with adults in both the community and acute settings? Do you have a passion for working with older adults to support their rehabilitation and transition back home? If yes, then this could be the post for you.

We are offering a rotational post for a Band 6 Occupational Therapist to rotate across the Complex Care Wards at University Hospitals Dorset (UHD) and Transition Team (Dorset Healthcare).

The Complex Care Wards, currently situated at Poole Hospital offer a fantastic opportunity for developing skills in frailty and complex case management. You will be encouraged and supported to enhance your practice and develop extended skills, working with our highly regarded, innovative and motivated older person's team in the hospital.

The Transition team provide therapy wraparound to the Discharge to Assess (D2A) beds located in East Dorset. The D2A beds form part of the Dorset System's core offer, a bedded option for those patients that are not yet safe to return home with support and require a period of further rehabilitation and assessment. The team works closely with social care colleagues to support the assessment and timely discharge of patients.

This post will enable you to develop your OT skills in both acute and community settings and form a greater understanding of the acute to community transition and the role therapy plays within this.

Main duties of the job

You will be responsible to autonomously carry a full clinical caseload of patients, using evidence-based and client centred principles to assess, plan, implement and evaluate specialist interventions.

You will need to be confident in the discharge to assess process and be able to work with therapy colleagues, ward-based MDTs, social services, care home providers, Bournemouth and Poole Hubs and SPA teams to plan and facilitate discharges. You will need to have prior experience in the assessment and prescription of equipment and be able to write comprehensive manual handling care plans.Having an understanding of the mental capacity act and its relevance and impact on the discharge process is crucial to this role.

You will be expected to work in a team of occupational therapists, physiotherapists and rehab assistants. Supervise OT students on practice placements, provide leadership for assistants through supervision and appraisals, maintain timely and accurate clinical records and statistical information as required. You will need to be willing to work weekends on a rota basis to support the 7 day therapy service.

As this post is not based in one location being a car driver and having access to a vehicle is essential.
