Are you a qualified Mental Health practitioner looking for a challenge. The Poole Adults Community Mental Health Team are looking for a Community Mental Health practitioner to join the team and be at the forefront of innovation as part of the mental health integrated community care project.
As a Mental Health practitioner, you will provide a comprehensive accessible and responsive mental health service to adults and their families within a defined locality. This is primarily a care-coordination role, where you will be responsible for supporting patients on their journey of recovery. You will be a committed, dynamic, motivated professional with excellent communication, organisational skills and the ability to prioritise your work. You should have recent experience of working in a mental health environment and be able to apply the philosophies of engagement and recovery in your work with service users.
You will work as part of a well-established supportive team made up of Psychiatrists, Nurses, Psychologists, Occupational therapists, carers officer, Employment Specialists, Physical Health Support Workers, Social Workers, Administration and Support Time Recovery Workers. We offer peer support and regular supervision to enable you to feel safe in your practice whilst further developing your skills.
Main duties of the job
To provide a comprehensive accessible and responsive mental health service to Adults and their families within a defined locality.
To support the team and locality leader in implementation of the Dorset Healthcare Mental Health Strategy.
To continually develop clinical, team and service evaluation and delivery of service.
To contribute to effective multi-disciplinary working within the community mental health service and to build up effective liaison with community, primary, and other secondary and tertiary services/agencies within the locality to provide holistic care based on the individual's need and service development.
This is a full time role, 37.5 hours, based at Alderney Hospital, you will need to have a valid driving licence and use of a car, as you will be required to assess and monitor patients in their own homes and within other community settings.
