Job summary
Come and join our Dorset Stroke and Neuro Service serving the West and North of Dorset. We have recently been awarded significant investment in Dorset to develop Stroke and Neuro Services across Dorset to meet the National Stroke Service model published by NHS England last year (particularly with regards to Integrated Community Stroke Services- ICSS).
This is a really exciting time as we significantly expand our existing services and embed innovative ways of working to improve clinical outcomes for patients following stroke or neurological illness or injury. Importantly this once in a generation investment increases our team threefold and significantly extends the time patients can access the service moving to a needs led, rather than time limited service. Our integrated community stroke and neuro team is AHP lead and will provide various pathways of care including supported self management, advice and signposting, intensive rehabilitation (ESD), complex management and reviews.
Our team expansion includes nursing, social work, neuropsychology and voluntary sector roles, as well as apprentices, rotations across the acute and community pathway and opportunities for advanced practice. Over coming years we aim to further bring together a range of specialists working across the fields of stroke and neurology in Dorset, meaning our integrated team will have access to a wide range of specialists with many years of experience.
Main duties of the job
We have a programme initiated within Dorset developing communities of practice regarding psychological care and motor relearning, which post holders can join.
This role is for a band 4 assistant rehabilitation practitioner in physical health working alongside OT's, physio's and SLT's carrying out physical, cognitive and speech and language exercises and the main duties of the role are:
To be responsible for a specific caseload
To work effectively as a member of the community stroke and neuro service
To support transition of people from in-patient rehabilitation to community rehabilitation
Provide a flexible, client centred intervention to promote rehabilitation in their own homes and other community settings.
To work with people to identify goals, contribute to the care planning process and the evaluation of interventions.
Assess for and issue specific equipment to facilitate independent living.
To undertake holistic six month stroke reviews in clinic and in patient's home
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
