Job summary
We looking to recruit two experienced and motivated Therapy Assistant Practitioners within the Transition therapy team in Poole. The roles will be varied and exciting and include community follow up with patients in their own home following a Patients rehabilitation journey from elderly Rehabilitation into the community.
Due to expansion of the team, we are excited and pleased to announce this opportunity for two Band 4 Therapy Assistant Practitioner posts (full time or part time hours considered). The successful candidates must have attained or be willing to work towards the required NVQ level in Health and Social Care.
You will require a car for use at work, to enable you to work autonomously in the community to visit patients, whilst adhering to local and Trust guidance on lone working.
This is an ideal opportunity if you have a keen interest in the patients journey through rehabilitation, working with patients from discharge form the acute hospitals all the way through to home-based exercise programmes.
Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.
Main duties of the job
The role will involve patient assessment, goal setting, implementation of evidenced based treatment and management strategies, and autonomously managing your own caseload In elderly rehabilitation. Primarily the successful candidate will work towards discharging the patients from inpatient rehabilitation setting through to home-based rehabilitation.
We offer employment in a dynamic team, of Rehabilitation Assistants, Physiotherapists and Occupational Therapists who work as part of a wider MDT.
The successful candidate will gain knowledge and experience alongside a range of colleagues, within a mutually supportive team. They will benefit from regular in-service training, and support through supervision and annual Professional Development Reviews, and contribute to the training and supervision of rehabilitation assistants.
You will be required to carry a clinical caseload of patients within Community hospital rehabilitation ward settings, D2A bedded units as well as within the patient's own homes following discharge. You will be responsible for ensuring that patients benefit from high quality, evidence-based interventions, which you will deliver and record according to professional standards and legal requirements.
