Job summary

An exciting opportunity has arisen working within the Transition therapy team for a Band 6 Occupational Therapist with previous orthopaedic and discharge planning experience.

The role includes rotation between ward based therapy within the COHO an into the community covering D2A beds, and seeing patients in their own homes.

Full and part time hours would be considered, However a minimum of 22.5 hours will be required.

We offer employment in a dynamic team, of Rehabilitation Assistants, Physiotherapists and Occupational Therapists, working as part on a wider Multidisciplinary team across the wards and community.

The successful candidate will gain knowledge and experience alongside a range of colleagues, within a mutually supportive team. They will benefit from regular in-service training, and support through supervision and annual Professional Development Reviews, and contribute to the training of more junior staff. You will be required to carry a clinical caseload of patients with a wide variety of conditions, ensuring that patients benefit from high quality, evidence based interventions, which you will deliver and record according to professional standards and legal requirements. The role includes patient centred; Assessment, treatment and discharge planning including equipment provision and use of the Discharge to assess model.

Main duties of the job

This Rotational post allows a mix of inpatient and community therapy working.

You will be responsible to autonomously carry a full, clinical caseload of patients, using evidence-based and client centred principles to assess, plan, implement and evaluate specialist interventions. You will need to be confident in the discharge to assess process and work with Social services, CHC, Poole Hub and SPA teams to plan and facilitate discharges. You will assess for and prescribe appropriate equipment and write comprehensive manual handling care plans.

You will supervise OT students on practice placements, provide leadership for assistants through supervision and appraisals, maintain timely and accurate clinical records and statistical information as required.
