Job summary
Healthcare Support Worker for the Diabetes Service based in Weymouth and Portland.
As a Health Care Support worker (diabetes), you will be working with clinicians in our locality team to support patients with diabetes to remain as healthy and independent as possible. You will be working in people's homes, as part of a locality based diabetes service, with support from nurses, doctors and other clinicians. Successful candidates will be willing to learn new skills and flexible to meet the needs of the service.
This post is Part time, 22.5 hours per week. Core hours for this role are Monday to Friday 9am-5pm
Main duties of the job
As part of Dorset's Accountable Care System journey, six million pounds are being invested in Dorset's Integrated Community and Primary Care Services.
The aim is to develop an integrated workforce within localities, with a strong focus on partnership working. Services will be based upon the needs of people in each locality, and will focus on providing;
- Services for people with high intensity and complex needs,
- A proactive approach to care for people with medium intensity needs caused by long term health problems
As part of this investment, an exciting opportunity has arisen for enthusiastic individuals with experience in either health or social care to join our forward thinking team in Weymouth as Health Care Support Workers.
In line with the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 anyone applying for a role which requires the post holder to work within a CQC registered care home after 11 November 2021, will be required to provide evidence that they are fully vaccinated against COVID-19, unless medical exemption applies. Evidence of vaccination status will only be accepted via the NHS App or Website.
