You'll work shifts between 8am and 9pm, including weekends, and play a vital role in early identification of deteriorating patients, all while developing your skills through continuous professional development.

Main duties of the job
- To provide high quality care to clients under the indirect supervision of Team manager within the community bridging service.
- To support clients with activities of daily living in a clients home
- Ensure high standards are maintained in a safe environment, which promotes equality and sensitivity for all individuals
- To participate in continuous professional development and other team activities
- Carry out a range of duties within agreed competency skill set e.g. meal prep
- To ensure that all patients on the caseload are safe.
