Heathlands Care Home is seeking a proactive Senior Physiotherapist to join our dynamic community rehabilitation team, supporting patients to achieve meaningful recovery, independence and improved quality of life.
About the Role
As a Senior Physiotherapist at Heathlands, you will undertake holistic and comprehensive physiotherapy assessments and deliver individualised treatment plans for patients within a community rehabilitation setting. Your expert clinical skills will contribute to high-quality, patient-centred care, ensuring safe and effective rehabilitation pathways.
You will work collaboratively as part of a skilled interdisciplinary team, supporting excellent day-to-day service provision and contributing to continuous service development.
Main duties of the job
- To provide holistic PT assessment, goal setting, individualised treatment programmes, care planning/delivery, evaluation, and discharge of patients
- To provide comprehensive environmental and functional assessments and treatment programmes as required ensuring the progression of the patient to their optimal level.
- To provide comprehensive manual handling assessment and care plans for inpatient staff to follow, ensuring effective and safe manual handling on the ward.
- To identify, assess and manage risk.
- Where appropriate to take responsibility for the application of new skills to rehabilitation programmes.
- To provide line management and supervision to junior staff and students as appropriate.
- To maintain excellent clinical practice through the CPD process, and taking part in relevant research, audit and development activity as directed.
- To work closely and jointly at times with Occupational Therapy and Nursing colleagues to ensure a holistic and effective care plan is provided to the patient.
- To be responsible for the day-to-day management and prioritization of your caseload and be able to respond to urgent referrals and an unpredictable workload.
- To be able to manage potentially stressful, upsetting, or emotional situations in an empathetic manner.
