We are pleased to offer an exciting opportunity for a Senior Community Physiotherapist to join our dedicated and supportive Community Therapy Team.
Specialising in long term disease management including, PD, MS and dementia, long term rehab, complex equipment, falls and frailty you will be supported by your team's MDT and B7 PT and OT Team Leads. You will receive regular supervision and be supported with a comprehensive induction to the team and community.
You will manage your own caseload, get involved in service development and provide clinical & operational supervision to junior staff. We will support you with your ongoing CPD including specialist study days, Masters modules in relevant subjects and clinical supervision to name but a few. We are proud of our accomplishments, our strong MDT approach and our constant evolution and development.
The wider Intermediate Care Team is a 7 day service, providing input between the hours of 8am-9pm, however, Community Therapies largely functions across an 8am - 6pm working pattern, Mon-Fri.
Ability to drive is essential for this Community role.

Undertake comprehensive clinical assessment, diagnosis, and treatment of patients on the caseload, including those with acute, complex, and/or chronic conditions, taking into account varied diagnoses, prognoses, and rehabilitation potential.

Provide evidence-based interventions in line with best practice, including public health education, falls prevention and management, chronic disease management, rehabilitation, and equipment assessment and prescription.

Develop person-centered rehabilitation goals and care plans collaboratively with patients, and where appropriate, with families and carers, to support functional independence and enable patients to remain safely at home for as long as possible.

Deliver care as part of an integrated multidisciplinary team, contributing to and promoting the inter-disciplinary working model within the intermediate care service.

Provide clinical input, advice, and support to colleagues across disciplines to ensure high-quality, coordinated, and holistic care.

Ensure accurate documentation and communication of patient progress, treatment outcomes, and care planning decisions.

Actively contribute to health promotion, early intervention strategies, and admission avoidance initiatives in the community.

Maintain up-to-date clinical knowledge and skills through continuing professional development, supervision, and participation in training and audit.
