An exciting opportunity has arisen to join the Trust as the Macmillan Palliative and End of Life Care Team Leader in Wexham Park Hospital. This post encompasses a combination of clinical, managerial and leadership responsibilities. This is a permanent post co-ordinating and delivering the vision of the 7 Day Hospital Palliative Care service.
The successful applicant will work as part of a well-established Palliative Care and EoLC multi-disciplinary team who provide specialist advice and support to patients, their relatives and members of the health care teams within the hospital and community. Applications are invited from a Nursing & Midwifery professional background who have worked at a senior level in palliative care. You should also have a Degree in Nursing, a recognised palliative care qualification, advanced physical assessment skills, be a non medical prescriber with excellent communication skills.
The postholder will be proactive in leading service redesign in palliative and end of life care within the hospital in order to make services responsive to changing needs. You will work closely with the Trust Lead for EOLC & and the Palliative Care MDT.
Main duties of the job
Provide leadership and support for the Palliative and End of life Care team to the Wexham Palliative care team.
Act in a consultancy role as an advanced clinical practitioner in palliative care, offering specialist advice, guidance and supervision to clinical staff, patients and carers
The post holder will act as an expert in palliative and end of care and advise on care for patients with advanced malignant and non malignant disease from time of diagnosis and throughout the trajectory of their illness
Lead developments in practice consistent with the patient care priorities set out nationally and locally. Develop new roles and redesign services in line with best practice and evidence based care.
