Job summary
The post holder is asked to provide skilled and competent care to meet patient's anticoagulation needs involving other members of the health care team as appropriate.
- Ensure the provision of continuity of care.
- Assess and evaluate, with patients, the effectiveness of the treatment and care provided and make changes as needed.
- As a leader in the field anticoagulation, make sure that each patient'streatment and care is based on best practice and evidence.
- Provide inpatient and outpatient anticoagulation services.
Main duties of the job
A new and exciting opportunity has arisen for a part time pharmacist or part time registered nurse to join our enthusiastic and specialist Anticoagulation team base at Wexham Park Hospital.As well as being responsible for providing a pharmacist and nurse led service, the successful candidate will be expected to complement the existing team and contribute significantly to the ongoing development within our field.
We require someone who is ready for a change and to assist us in maintaining our excellent Anticoagulation Service. The role involves providing Warfarin dosing, running clinics and management of DOAC / LMWH. You will work independently within the Anticoagulation Team, be self-motivated and require excellent communication, IT and keyboard skills.
Liaison and communication with other professional groups, agencies and the user group, both within the hospital and in the community, will be an essential component of the role.
