Job summary

We are looking for a community nursing sister/charge nurse or a District nurse for the Yateley and Aldershot community nursing teams to lead from the front to ensure the patients on the community nursing caseload are given holistic high quality care whilst ensuring unnecessary hospital admissions.

This is a rewarding role, working in cohesion with the other community sisters/charge nurses and team leaders to deliver care to patient's in their homes.

Working in the community you will be expected to care for patients who are housebound or require the services of the community nursing team for nursing needs such as administration of medication, leg ulcer and pressure ulcer care. The community nursing team also care for patients who are receiving treatment for cancer or who are terminally unwell and wish to be at home for their end of life care. Car driver is essential with a U.K Full driving licence with access to a vehicle for work. Mileage is claimable via the trust expenses system.

Main duties of the job

  • The role of the B6 Community Sister/District nurse/charge nurse to manage the day to day running of a community nursing team in conjunction with the Team Leader along with clinically visiting patients in their homes, providing excellent holistic care.
  • The post holder will be responsible for ensuring the community nursing team are functioning at a high level providing excellent patient care for those patients who are on the community nursing team caseload.
  • The post holder will be part of a supportive team of other B6 across the other teams covering the North east of Hampshire providing a 7 day service covering the hours of 8am to 8am.
  • The post holder ideally has been working in the community setting previously for a minimum of four years, or who feels their current skills can be transferred to the community setting.
  • The post holder needs to have completed the practice assessor/Mentorship course.
  • The post holder will be working in an autonomous environment and be able to make clinical decisions on their own whilst in the patients' home.
