Job summary
As a Community Multiple Sclerosis nurse/practitioner you will be working alongside a B7 Community Nurse Specialist to actively manage the community caseload who have multiple sclerosis.
You will be expected to attend outpatient clinics, complete home visits covering a large area across the north east of Hampshire and Surrey.
You will be supported by the B7 Clinical nurse specialist with great opportunities to develop additional skills and undertake further continuous professional development.
You must be a car driver with access to a vehicle for work, mileage expenses are claimable via the trust expenses system.
Main duties of the job
To visit patients in their own homes who have a MS diagnosis who have been referred to the community MS team.
To undertake outpatient clinic appointments in various settings.
To undertake virtual consultations.
Working in conjunction with the B7 MS nurse develop the service.
Liaise with GP's, PCN's.
Refer to other services when appropriate or clinically indicated for the patients on the caseload.
Link with the secondary care, nurse consultant and neuro team.
