A sensational opportunity has arisen for B4 Associate Practitioners to join the well-establish, innovative, and dynamic Respiratory Services as we expand our successful teams to provide Acute Respiratory Virtual Ward.

The Virtual Ward is an exciting new service offering home based care to patients who would otherwise be admitted to hospital.The successful candidate will play an integral part in delivering care to complex respiratory patients and will be expected to understand the management of multiple respiratory conditions and be aware of local and national guidelines related to these.

The award-winning teams are based in the Chest Clinic at King Edward VII Hospital in Windsor and Farnham Community Hospital, but we see patients as hospital in-patients, in clinic and in their own homes.

The role is a predominantly Monday - Friday with the hours being 9-5, although if the service demands you'll be expected to work flexible, unsocial hours and weekends.

Main duties of the job

Carrying out assigned tasks as part of clinical team in support of registered nurses and to undertake some aspects of care provision without direct supervision. Ensuring the safety and well being of patients, and always working in a manner that promotes dignity and human rights through the adoption of person-centered care principles.

The postholder will provide a seamless service to patients with supervision from a registered nurse whilst ensuring appropriate guidance and teaching of healthcare assistants. The post holder will demonstrate a level of underpinning knowledge to practice and educated to foundation degree level.

The post holder will support the registered nurses, medical staff and allied health professionals within the clinical environment. The role is flexible and adaptable to the changing needs of the care setting, and it is expected that the responsibilities will be developed and be agreed as services evolve.

Job responsibilities

  • Support the Specialist Nursing and Allied Health Care professionals to provide advice and support to respiratory patients in the community but will also assist the respiratory team at the acute hospital sites to facilitate early discharge onto the respiratory virtual ward.
  • In conjunction with the clinical Nurse Specialist and medical colleagues assist in the education required by patients prior to discharge, including the completion of appropriate bundles.
  • Work with other team members embracing the long-term conditions and virtual ward agenda thus preventing avoidable hospital admissions.
  • Under supervision initially the post holder will be expected to deliver patient education at both pulmonary rehabilitation classes and patients homes when required.
  • Under the direction of a registered Nurse / Physiotherapist with or without direct supervision to assist in assessment and safety visits of patients being considered or established on home oxygen. Training will be provided for this.
  • With the appropriate training become proficient and competent to work in a specialist respiratory team as deemed appropriate to their specific role.
  • Support and teach patients to self-manage their condition where possible and empower carers to actively participate in the treatment plan.
  • Adopt a public health approach to all areas of practice to reduce ill-health in particular stop smoking support.
  • Have a flexible approach to working pattern and be willing to work unsocial hours and weekends if the service requires.
