Job summary

We are looking for a proactive Respiratory Nurse/Practitioner to join our team in an exciting and busy role!

The Respiratory Nurse/ Practitioner is a nurse who is developing clinical, communication and inter-personal skills to be able to identify and support the needs of people with respiratory conditions in particular COPD, Asthma, ILD and home oxygen users and give care that recognizes social, cultural and spiritual differences within their lifestyle. The nurse has a developing clinical knowledge and interest in these specialist fields and works as part of a multidisciplinary team

Main duties of the job

As part of the Respiratory team the Respiratory Nurse will assist in the co-ordination and care of patients with respiratory illness to ensure they receive the highest quality care. The post holder will help with facilitating the discharge of appropriate respiratory patients from hospital, offer advice and assist in assessing the need for home oxygen and whenever appropriate prevent admission for respiratory patients.

They will also provide oxygen assessment and respiratory review to patients in the community manged on the acute respiratory virtual ward within East Berkshire/ North East Hampshire, Farnham and Surrey Heath. They will work under the guidance of a Clinical Nurse Specialist (CNS)

They will work as part of the team with the clinical lead in the development of the respiratory service towards a consistent and integrated approach to respiratory care within Frimley Integrated Care System.

Job responsibilities

COMMUNICATION

  • Develop and maintain high standards of communication, including liaising with other health care professionals and clinical lead to ensure the needs and safety of all respiratory patients are met.
  • Communicate regularly with the multidisciplinary Team to promote good liaison within the Team.
  • Under the guidance of the CNS, develop knowledge in respiratory care providing information to patients, health care professionals and the general public.
  • Be part of the team in discussing any referrals from GPs, Hospital Doctors and Chest-Physicians and patients, to decide on the appropriateness of the referral. Communicate and liaise with patients and their families, chest physicians, GPs, Ward Nurses and Doctors, Public Health Nurse, Health visitors and Community Nurses and other virtual ward providers.
  • Ensure effective communication by using interpreters when needed.
  • Maintain up to date patient written and electronic records.
  • Ensure that respiratory patients receive effective counselling /education. The progressive nature of their disease may involve supporting patients who receive bad news of a diagnosis and prognosis.
  • To develop an understanding of Trust and Board organisational structures and the structure of community services, and how we communicate within them.
  • Maintain accurate documentation and audit in line with FH and BHFT Clinical Governance Policy and NMC guidanc

CLINICAL

  • Accurately assess unwell patients with who have been admitted to the Acute Hospitals with the view to supporting discharge. This will involve reviews of their physical needs, their social and psychological circumstances. Discuss/report concerning clinical signs with the Nurse in Charge and the CNS.
  • Additionally, help support patients that have been identified by other health care professionals as needing to be monitored at home, thereby preventing their admission to hospital and caring for them on the respiratory virtual ward.
  • Visit patients in their homes to assess their progress; assist in the planning, delivery and evaluation of patient care during their period of acute illness and recovery.
  • Having reported outcomes, assist the CNS in safely discharging patients from the service.
  • Identify and rectify, when able, problems that arise during patients time with the service, liaising with appropriate personnel. Report these issues to the respiratory team.
  • Assist in providing a respiratory service consistent with the latest Department of Health recommendations and according to local needs.
  • Work towards developing competence in making efficient decisions within and on behalf of the service.
  • Participate in developing standards of care and monitoring clinical practice in partnership with the GPs, Respiratory team and Community Health Services.
  • As part of the complex approach to admission prevention, in some respiratory patients, help identify at risk patients and seek guidance as to how to intervene to reduce the need for admission.
  • To always adhere to the NMC Code of Conduct and Scope of Professional Practice.
  • To contribute to palliative care of respiratory patients as they approach end of life.
  • To provide oxygen assessment to those patients referred to the service within the clinic setting, gaining knowledge in considering the most appropriate modality, should oxygen be required.
  • To complete home oxygen reviews within the patients usual residence as per guidelines with guidance from the CNS
  • To develop competence in using the blood gas and CRP analyser and spirometry equipment.

  • Be proactive in reflection, self-assessment and development; participating in peer review as appropriate
