Community Nursing Sister/Charge Nurse

Job summary
The community nursing out of hours service is looking to recruit a community sister/charge nurse into the team to cover set nights from 20:00hrs to 08:00am covering Monday, Thursday and Friday with an additional Saturday per month visiting patients in the North east of Hampshire working in conjunction with HCRG as a night team.
The community sister/charge nurse will visit patients with a HCA as a joint visit, visiting patients who access the service throughout the night.
Ability to drive is essential, with a full U.K driving licence also with access to a vehicle for work.
Main duties of the job
To provide excellent clinical management for patients with complex care needs during the hours of 20:00pm to 08:00am who are at risk of deteriorating health that may result in declining quality of life who require nursing care in their own homes. This could be managing end of life care, catheter care or wound care.
You must display a professional attitude and be accountable for the leadership of the team in conjunction with another community nursing sister thus ensuring positive outcomes and high quality care to patients.
