Job summary
Cancer Pathway Trackers will b responsible for documenting and monitoring patients' cancer pathways to ensure accurate information is entered into the Cancer Tracking Database (Somerset Cancer Register), read and interpret clinic letters to determine patients' next steps, and escalate any delays in the pathways.
They will transfer key information from the Trust's Electronic Patient Record system (EPIC) into the Cancer Tracking System (Somerset Cancer Register) ensuring that Trust's standards are adhered to in relation to all documentation and data input.
Cancer Pathway Trackers will be responsible for monitoring patients following an urgent suspected cancer referral to ensure that patients are seen and investigated/diagnosed within nationally agreed parameters and that the necessary escalation and subsequent documentation is carried out for patients booked outside of those parameters.
They are to be responsible for alerting and escalating to the Shared Care Navigator Team Leader and Cancer Performance Manager when problems and delays arise.
Main duties of the job
- To work independently with minimal supervision and as part of a team prioritising activities to ensure workload is completed in a timely manner.
- To follow the departmental escalation process by informing the Cancer Services Team Leader if they become aware of any potential issues/breaches against the Cancer Waiting Times targets and update Somerset with all actions taken, contributing towards any potential breach reports.
- To utilise hospital systems, such EPIC and Somerset Cancer Register, to track and monitor patients appropriately through their suspected cancer pathways.
- Accurately record referrals onto Somerset from EPIC, ERS or referral letters. To chase images and relevant reports from other hospitals and upload to EPIC.
