Job summary

This is a rare opportunity: to be a founding member of a new MDT, establishing a new team in a new purpose-built intermediate care unit: Heathlands.

Heathlands is an exciting new venture in conjunction with Bracknell Forest Council to provide both intermediate and dementia care to Bracknell and the surrounding area. Frimley Health are pleased to be the official provider of the intermediate care unit, which is due to open in January 2022.

It is a 20 bedded unit providing patients with both step up and step down services.

Main duties of the job

As an experienced Physiotherapist you will be confident in assessing and treating patients, developing rehab and treatment plans and supporting patients to achieve their goals. With experience of goal setting, prescribing mobility equipment, group work and discharge planning you will be able to motivate, encourage and support patients with a range of functional interruptions.

You will have excellent communication skills, be a team player and have a special interest in service development. In return you will receive regular supervision, CPD opportunities and be supported to develop both your clinical and leadership skills.

As well as providing direct care and treatment to patients you will be a key influencer in the development of this team, building relationships and efficient pathways both internally and with community providers.

You will be expected to work across 7 days on a rota basis and will need a car for work purposes. There are full and part time roles available.

About us

Frimley Health NHS Foundation Trust provides NHS hospital and community services for around 900,000 people across Berkshire, Hampshire, Surrey and south Buckinghamshire.

We have three main hospitals - Frimley Park in Frimley near Camberley, Heatherwood in Ascot and Wexham Park near Slough.

In addition to our 3 main hospitals we provide integrated community services to north east Hampshire, Farnham and Surrey Heath in partnership with Virgin Care Services.

Our threecore values, and the behaviours that support them, guide everything we do and set out what we expect of our staff in the way they treat patients, visitors, service users and each other.

When you join our trust, you become part of the Frimley Family - one team who work together to deliver truly exceptional care to the local residents we serve. Thanks to the breadth of our clinical and community services, you'll find a diversity of career opportunities and pathways.
