Job summary

Our diabetes team is going from strength to strength, working together to transform the way we deliver care you our children and young people living with diabetes. With the support of the current MDT you will be bring your ideas to help us to be as accessible as possible to our patients.

You will be a budding PDSN with a genuine interest in the care of patients with diabetes. We will support you to undertake the appropriate training, which upon completion will enable you to progress to a PDSN role.

We will support your development when you join our team which will be led by a cross site Lead PDSN. You will be based at Frimley Park Hospital, but part of a wider nursing team which gives greater opportunities to learn and develop.

Main duties of the job

  • To review all children and young people admitted to hospital with a diagnosis of diabetes, with the aim to support early discharge so that they can be managed in their own home and reduce the likelihood of readmissions.
  • Toundertakecomprehensivehealthcareneedsofpatientswithdiabetes, changing treatmentplanasnecessary,includingassessmentofpatientswhenonan inpatient wardandatpostdischargethroughaclinicorvirtual visits.
  • To be skilled in the assessment and basic management of patients with diabetes, developing autonomous clinical decision making.
  • To provide appropriate information, education, advice and support particularly to the patients and parents.
  • To provide the nursing and support for children and families who have diabetes and/or experiencing complications, high HbA1c treatment managementi.e.pump starts

  • Relevant specialist post basic programme
  • First degree in nursing, or demonstrable experience at this level

Desirable

  • Completed / completing a Master's pathway
