Job summary

Community services and older persons and palliative directorate are looking to recruit a permanent, full-time or part time band 6 Community / District Nursing Sister / Charge Nurse to our supportive and hard working team in the Aldershot , Fleet and Yateley localities.

As a Community nursing sister/Charge nurse or qualified District Nurse for the community nursing teams you will need to have experience of caring for patients who have complex health needs, who require the services of the community nursing team in the patients own home.

In conjunction with the Team leader you will actively line manage the community nursing team and participate in the delivery of the service.

The Community nursing team provide a seven day 8am to 8am service which will include a variety of shift patterns which may also include some night shifts.

Due to the nature of the post the applicant must be able to meet the travel requirements of the post and hold a driving licence with access to a car.

Mileage expenses for home visits/training is included within this role.

We are able to offer CPD, via university courses, including inhouse modules with additional support from our practice development nurse.

You will be fully supported in this role from senior staff across all localities.

Main duties of the job

The main duties of the role are to :-

  • Manage the day to day running of the Aldershot/Fleet/Yateley community nursing team with the support of the Team Leader.
  • Visit patients in their homes providing excellent care such as wound management, pressure ulcer management, end of life care, catheter care, central line care.
  • Administer Insulin and other medications as prescribed by the patient's GP or following discharge from hospital.
  • Facilitate end of life for patients who are on the end of life pathway in conjunction with the wider MDT and other providers.
  • Complete off duty rota's for the community nursing team.
  • Line manage the community nursing team with support from the Team leader.
  • Facilitate discharges from the caseload.
  • Complete first visits for patient admitted to the caseload where possible.
  • Participate in training in house and by other providers to further develop your existing skills but also to facilitate your revalidation.
  • Be a role model for the Community nursing team ensuring that all policies are followed.
  • Attend MDT meetings and internal meetings with the Senior Community matron when required.
  • To support the junior members of the team with yearly appraisals, monthly one to one's.
  • Create a supportive learning environment for student nurses, student nursing associates.

Training on aspects of community nursing will be provided with support given for completion of competencies.
