Main duties of the job
We are looking for an enthusiastic and committed Consultant Anaesthetist with a special interest in Chronic Pain to join the team of Consultants in the Anaesthetic Department at Wexham Park and Heatherwood hospitals, Frimley Health NHS Foundation Trust. Anaesthesia and Chronic Pain are part of the Directorate of Anaesthesia, Theatres and Critical Care. Both ICU units have received the rating of Outstanding by the Care Quality Commission.
This is a post in anaesthesia and chronic pain. The anaesthesia component will be based at Wexham Park Hospital and the Heatherwood Hospital with the opportunity to subspecialise in Obstetrics, Regional, Trauma and colorectal anaesthesia dependent on individual interest and training. The chronic pain component will involve all three hospitals in the directorate (Frimley Park Hospital, Wexham Park Hospital and the Heatherwood Hospital). This post has arisen because of increased demand and retirements.

Job responsibilities
The successful applicant will be expected to take part in developing services within the Trust and to share the clinical, administrative and teaching work with other
consultants in the department. The post holder will join a multidisciplinary team providing an Outpatient/Chronic Pain service to the Frimley ICS Population (approximately 800,000 people).
The basis of the locum consultant post is 10 programmed activities which is split between 9 direct clinical care (DCC) sessions and 1 supporting professional activity (SPA). The clinical sessions will be allocated for chronic pain lists and clinics and provision of anaesthesia for various specialities at Wexham and Heatherwood Hospitals.The job will also involve 1 in 4 Saturday working including chronic pain lists and also providing anaesthetic services for orthopaedic trauma and elective operating lists.
The post holder will be expected to dedicate approximately half their sessions to chronic pain, of which there will be a mixture of outpatient pain clinics, theatre interventional pain lists, and the remainder to sessions in anaesthesia, acute pain sessions and weekend lists. There may be also be an expectation and opportunity to cover on calls in the capacity of a consultant covering theatres and obstetrics. The job plan will be reviewed on a regular basis with the Chief of Service/ Deputy Chief of Service. All changes would be by mutual agreement factoring the department and individual requirements. Changes having budgetary or contractual implications would also need the agreement of the Chief Executive.
So far as is consistent with the proper discharge of the above duties, the post holder undertakes to deputise from time to time for absent colleagues, this includes short periods of annual leave, sick leave, study and professional leave on a quid pro quo basis. Paid backfill is provided in exceptional circumstances. On occasions Consultants may be requested to perform additional duties in emergencies or unforeseen circumstances or convert SPA time to Direct Clinical Care.
Representative timetable/Job plan
The proposed timetable and PA allocation are indicative of the work required but is not rigid and will be flexible depending on service needs and the base specialty of the applicant. A formal job plan will be agreed between the appointee and Chief of Service on behalf of the Medical Director within three months of appointment.
The job plan will then be reviewed annually and covers all aspects of a consultants professional practice including clinical work, teaching, research, education and managerial responsibilities. It will provide a clear schedule of commitments, both internal and external. In addition, it will include personal objectives, including details of their link to wider service objectives and details of the support required by the consultant to fulfil the job plan and the objectives.
The total job plan is 10 PAs (9 DCC and 1 SPA) and is split approximately 50:50 in terms of pain and anaesthesia. The pain work will be across Frimley Health and take place at Wexham Park Hospital, Windsor, Heatherwood Hospital, Bracknell as main base but work at Frimley Park Hospital and Aldershot centre for health may also be required. This will be negotiated taking into consideration requirements for the pain work, vacant theatre and clinic slots.
The DCC component of the job will also involve 1 in 4 Saturday working including chronic pain lists and also providing anaesthetic services for orthopaedic trauma and elective operating lists. There will also be acute pain sessions based at Wexham Park Hospital.
There will be access to secretarial support, IT systems and the Trusts mentoring scheme.The multidisciplinary pain team comprises consultant doctors, a nurse consultant, specialist nurses, an extended scope physiotherapist, specialist physiotherapists, a pain fellow, OT and psychologists. We are supported by an excellent admin team in both the anaesthetic office and the pain clinics. The service can offer a holistic approach to pain management supported by multidisciplinary working that includes the provision of interventions, pharmacological management, physical therapy, and pain management programs. As an Outpatient/Chronic Pain Service we work closely with our Inpatient/Acute Pain Services and the nursing team is integrated across the Trust.
Trauma (Wexham Park Hospital)
Wexham Park Hospital is a busy DGH trauma and orthopaedics centre and is part of the regional TARN (Trauma & Research Network). Wexham Park Hospital is a centre of excellence for outcomes in Fractured NOF patients. It is amongst the top 10 trusts in the country in outcomes and BPT (Best Practice Tariff) measures. Our adjusted mortality rate is well below national average. The service is covered by 8 Anaesthetic consultants along with trainees and SAS grade doctors. There are two dedicated trauma theatres including dedicated weekend theatre capacity supported with HDU/ICU and extended recovery facilities. The service is also complimented by 2 trauma nurse practitioners, a trauma Coordinator and an Ortho-geriatric consultant. There is a dedicated consultant led multidisciplinary Fractured NOF Network.
Successful candidates will have the opportunity to work along with the present consultant anaesthetists covering Trauma anaesthesia. There will be ample opportunities to participate in Quality improvement projects such as FIB (Fascia Iliaca Block) service setup. There will be opportunity for taking part and teaching in the WPH Regional Ultrasound course and teaching/training of trainees.
The Critical Care structure has recently been reviewed and modified to reflect cross-site (Wexham-Frimley) working to include a cross site Consultant ICU lead and Head of Nursing for Critical Care. The cross-site clinical governance meetings and MDT half-days are held regularly.
Management arrangements
The Trust is organised into Clinical Directorates.
The Directorate of Anaesthesia, Theatres and Critical Care spans all three sites and is led by:

Teaching, audit and research
The Anaesthetic departments offer an active teaching programme for junior anaesthetists. We get trainees from Oxford deanery, part of Heath Education Thames Valley. Difficult airway workshops are held annually as well as courses relating to Obstetrics, Human Factors and Regional anaesthesia. Consultants also provide tutorials for those junior staff preparing for examinations. Participation in Clinical Audit is a requirement and an ongoing activity in this department. Research activity is encouraged and supported by the Trust Research and Development Department. Attendance at the monthly Educational day is expected with the exception of those providing emergency care cover.
There is a focus on the use of simulation based training with SIM suites and SIM fellows.
Continuing Professional Development
The post holder will be expected to keep themselves fully up to date with their relevant area of practice. Professional or study leave will be granted at the discretion of the Trust, in line with prevailing Terms and Conditions of Service, to support appropriate personal development needs. The post holder is expected to meet the Continuing Professional Development requirements of the RCoA.
The Trust currently makes a financial contribution in support of these activities up to an agreed limit, which is currently £900 per annum.

The appointee will be expected to live within 10 miles or 30 minutes travelling time of their base. A relocation package is available to the successful candidate.
