Senior Physiotherapist - Admission Prevention Team

Job summary

The post holder will work within the A&E department based at the Princess RoyalHospital. The post holder will act as an ambassador for Sussex Community NHS Foundation Trust and play a key role in supporting at the front door of A&E in preventing hospital admissions.Utilising their knowledge of the community services and clinical skills the post holder will work closely with the A&E staff to ensure seamless, safe and effective discharge plans are in place for the person to return back to their usual place of residence as soon as possible, preventing admission to hospital.The post holder will play a key role in using data from incidents, feedback and observation to identify potential improvements or changes within the service which will support effective safe discharge.To treat all patients with respect and courtesy, maintaining dignity and respecting individual needs and differences, both physical and cultural.

Main duties of the job

The post holder will have excellent clinical decision-making skills that willsupport the A&E staff in the management of discharging patients into thecommunity and community beds.The post holder will support with the induction of new staff.The post holder will attend MDT meetings, and support A&E staff with complex discharge functions as required.To provide training and education to A&E staff around what support community services can provide and support in discharge management & processes.

To ensure records and data are maintained in a timely manner and in line with information governance and data sets required for the service.
