Job summary

Chanctonbury PCN is seeking an experienced and motivated Clinical Nurse lead, to coordinate, and support our established team in delivering high-quality care to patients who are unable to leave their homes.

Our care model is person-centred and collaborative, ensuring that every patient receives holistic care tailored to their individual needs. Working closely with local services, you will play a key role in shaping community nursing and driving improvements that make a real difference to people living in Chanctonbury and surrounding areas.

Main duties of the job

Chanctonbury PCN is seeking an experienced and qualified Community Clinical Nurse Leadwith advanced practice credentials, strong clinical expertise, and proven leadership skills to join our team.

In this pivotal role, you will:

  • Coordinate complex care for patients in the community, including comprehensive assessment and management of long-term conditions.
  • Apply case management principles to support patient self-management, promote independence, and reduce hospital admissions through robust contingency planning.
  • Lead on staff management and development, including supervision, appraisals, and mentoring of junior team members.
  • Act as an educator and preceptor, driving professional growth within the nursing team and championing innovative self-care strategies for patients.
  • Foster collaboration across services to ensure high-quality, person-centred care for our local population.
