Job summary

Community Liaison Nurse - Admission Prevention Team

An exciting opportunity has arisen within the Admission Prevention Team based at the Royal Sussex County Hospital for an experienced nurse. The aim of the team is to triage and assess patients that come into the emergency department by ambulance, with a view to preventing admission to hospital.

Main duties of the job

The post holder will work within the A&E department based at the Royal Sussex County Hospital. The post holder will act as an ambassador for Sussex Community NHS Foundation Trust and play a key role in supporting at the front door of A&E in preventing hospital admissions.

Utilising their knowledge of the community services and clinical skills the post holder will work closely with the A&E staff to ensure seamless, safe and effective discharge plans are in place for the person to return back to their usual place of residence as soon as possible, preventing admission to hospital.

The post holder will play a key role in using data from incidents, feedback and observation to identify potential improvements or changes within the service which will support effective safe discharge

Essential

  • Registered Nurse, with current NMC registration
  • Post registration study or qualification

Desirable

  • Mentorship qualification
  • Leadership qualification

Experience

Essential

  • Minimum 3 years' experience in Band 5 role
  • Experience of working within a community or hospital setting
  • Excellent interpersonal skills
  • Experience of working with multi-disciplinary teams

Desirable

  • Experience of providing supervision/appraisal

Skills and Knowledge

Essential

  • Evidence of a range of advanced clinical assessment skills appropriate for the role
  • Advanced communication skills including difficult conversations, negotiation, managing conflict
  • Promotes patient centred care including self-management
  • Demonstrates application of evidence-based practice
  • Knowledge of best practice discharge planning processes
  • IT literate
  • Understanding and knowledge of current issues in the NHS
