Urgent Community Response - Advanced Clinical Practitioner

Job summary

It's an exciting time for our Urgent Community Response (UCR) Service including the General Virtual Wards Caseload, as we continue to build on our successful model. UCR services work with patients in their homes across West Sussex communities to avoid hospital admissions and support those being discharged home.

The Service will provide clinical interventions (including intravenous therapy) from a team of Medics, Nurses, Paramedics, Physio and Occupational Therapists for admission avoidance pathways (2 hour response and general virtual wards), in line with thenational specification for UCR

Operating 365 days per year, the service receives referrals from a range of health and care professionals and triages, assesses, and delivers care to the patient over several days to ensure the patient can remain safely at home.

Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm.

We are looking for an Advanced Clinical Practitioner (Nurse or Allied Health Professional) to join us, so if you're a dynamic, patient focused individual looking to take a lead role in a team that provides cutting-edge, excellent acute community care, we'd love to hear from you!

Main duties of the job

The postholder will be required to work autonomously as an Advanced Clinical Practitioner, using these skills to assess, investigate, diagnose, treat and manage patients who may have co-existing and diverse factors making management complex. They will develop, utilise and maintain skills beyond the scope of normal practice (for example advanced physical assessment, diagnostics and prescribing). Within the role there will be a requirement to plan and direct the management of patients including accessing services within the multi-disciplinary team and wider health, social care, education, vocational and voluntary sector services as appropriate to the role. The postholder will lead on the strategic development of Trust-wide services in their specialist area, working with other strategic and clinical leads as appropriate. They will also lead and participate in education programmes designed to support other clinical staff skills within their speciality.

The postholder will provide peer support to clinical staff working with patients from the direct caseload or those involved in the management of patients. They will assist in the planning, co-ordinating and development of any newly implemented service plans as well as being responsible for ensuring that relevant legal and regulatory frameworks are acknowledged through local governance procedures.

Qualifications and/or Professional Registration

Essential

  • MSc in Advanced Clinical Practice or equivalent at MSc level in clinical practice at level 7.
  • Hold a registered Health Care Professional qualification/degree appropriate for ACP role
  • Current Registration with NMC / HCPC
  • Evidence of recent CPD in relevant field
  • Independent/Supplementary Prescribing (HCPC V300) or Advanced Medicines Management module
  • Advanced Physical Assessment course Level 7
  • Coaching or Mentoring qualification

Desirable

  • Teaching qualification (eg PGCert)
  • Post graduate leadership qualification

Experience

Essential

  • Substantial post registration experience in relevant clinical field
  • Experienced in Leadership/Management at band 7 or above
  • Experience of teaching and mentoring staff at all levels across a multi-disciplinary team
